How to join
If you want to manage by your self your medical records go to on line subscribe form and wait for our communication. You will pay only after your subscription is accepted.
Make the payment by safe system credit card payment on line.
Send us the Form , together with the receipt for payment for one year's subscription or more.
Your subscription will be confirmed within a few days (usually by e-mail) together with the address of your membership section, where documents, including a recent photo, should be sent.
The quality of the photo archive depends on the quality of the original photos.
The subscription takes effect 60 days after confirmation.
Documents should be sent all together within a fortnight by mail or e-mail.
Upon request, we can authorize you to work directly in your own account. In this case, you don't have to send us your medical file or any photos. In addition, you will be completely free to update your documents whenever you see fit.
